The Electronic Health Record (EHR) is a device who will record of patient health medical story, It is a record in digital format that is capable of being shared across different health care settings, by being embedded in network-connected enterprise-wide information systems. Included in this information are patient demographics, progress notes, problems, medications, vital signs, past medical history, immunizations, laboratory data and radiology reports.

For example, the EHR can improve patient care by :

1. Reducing the incidence of medical error by improving the accuracy and clarity of medical records.
2. Making the health information available, reducing duplication of tests, reducing delays in treatment, and patients well informed to take better decisions.
3. Reducing medical error by improving the accuracy and clarity of medical records.

Its purpose can be understood as a complete record of patient encounters that allows the automation and streamlining of the workflow in health care settings and increases safety through evidence-based decision support, quality management, and outcomes reporting.
